A. General Liability Coverage $1,000,000 Minimum
B. Workers Compensation Coverage $500,000 Minimum

D. It is the subcontractor’s responsibility to forward updated COI’s to Meridian should coverage expire or have a carrier change.

INVOICING GUIDELINES
1. Invoices must be submitted with a SUBCONTRACTOR AGREEMENT which is completed and agreed upon prior to work being performed.
2. Invoice must be submitted with a LIEN WAIVER
3. Meridian writes checks once a month – the 15th (INVOICES RECEIVED BY THE 1ST OF THE MONTH ARE PAID ON THE 15TH)
4. It is best to use a numbered invoicing system
5. Always include the following on invoices
A. Job Site Address
B. Project Managers Name
C. Description of work performed
D. Amount Due
